RECORD OF PROCEEDINGS
AIR FORCE BOARD FOR CORRECTION OF MILITARY RECORDS

IN THE MATTER OF:	DOCKET NUMBER: BC-2013-05709
		COUNSEL: NONE
		HEARING DESIRED: NO
________________________________________________________________

APPLICANT REQUESTS THAT:

His military personnel records be corrected to reflect his
correct Social Security Number (SSN).

________________________________________________________________

APPLICANT CONTENDS THAT:

His SSN is XXX-XX-XXXX instead of XXX-XX-XXXX.

In support of his request the applicant provides a copy of his
Social Security Account Number card and a DD Form 215,
Correction to DD Form 214, Armed Forces of the United States
Report of Transfer or Discharge.

The applicant’s complete submission, with attachments, is at
Exhibit A.

________________________________________________________________

STATEMENT OF FACTS:

According to the applicant’s DD Form 4, Enlistment Record· Armed
Forces of the United States, on 7 Jun 65, the applicant enlisted
in the Regular Air Force for a period of 4 years. The DD Form
4 does not reflect his SSN, only his service number.

According to the applicant’s DD Form 214, on 6 Dec 68, he was
released from active duty and credited with three years and six
months of total active service. The DD Form 214 reflects his
SSN as XXX-XX-XXXX.

On 5 Nov 69, a DD Form 215, Correction to DD Form 214, Armed
Forces of the United States Report of Transfer or Discharge,
changed his service number from XXXXXXXX to XXXXXXXX.

________________________________________________________________

AIR FORCE EVALUATION:

AFPC/DPSIRP recommends denial, indicating that AFI 36-2608,
Military Personnel Records System, Table A7, Note 1, states “Do
not correct records of former members unless evidence proves the
SSN used while serving with the Air Force was erroneously
recorded.” A review of the applicant’s personnel records
revealed all documents containing his SSN have it documented as
“XXX-XX-XXXX” and this was the SSN he presented for enlistment.
There was no change to his SSN during his enlistment and it does
not appear to have been erroneously recorded.

The complete DPSIRP evaluation is at Exhibit C.

________________________________________________________________

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

The applicant provided a printout from the Social Security
Administration that reflects his SSN as “XXX-XX-XXXX” (Exhibit
E).

________________________________________________________________

THE BOARD CONCLUDES THAT:

1. The applicant has exhausted all remedies provided by
existing law or regulations.

2. The application was not timely filed; however, it is in the
interest of justice to excuse the failure to timely file.

3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. We note
that DPSIRP recommends denial stating that the SSN reflected in
his records is what he presented for enlistment and it does not
appear to have been erroneously recorded. However, after a
thorough review of the evidence of record and the applicant’s
complete submission, it is our opinion that the applicant’s SSN
may have been erroneously recorded at the time of his
enlistment. In this respect, we note the applicant’s enlisted
document (DD Form 4) does not contain a place to record a SSN,
only the service number, which was also erroneously recorded.
The only documents reflected in his records that contain the
contested SSN are the documents issued at the time of
separation; all the other documents we reviewed contained the
service number. Therefore, we find it in the interest of
justice to recommend the applicant’s DD Form 214/215 be changed
to match the Social Security Administration’s records.
Accordingly, we recommend his records be corrected as indicated
below.

________________________________________________________________

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air
Force relating to the APPLICANT, be corrected to show the social
security number on the DD Form 214, Armed Forces of the United

States Report of Transfer or Discharge, reflect XXX-XX-XXXX
instead of XXX-XX-XXXX.
